Some suggestions:

1- Read the messages in the thread you've hijacked,
2- Use the Message area to state your question/issue - the Subject line is
limited & doesn't fully display,
3- Look in the Project Gallery for templates,
4- Always specify your version of Office & OS X

I cannot find or bring forth or download or make appear the plate to the right of a document (which has underline on it.
