I received, as per usual, a number of junk e-mails today. One of them
was the usual fake "delivery unsuccessful" spam. This one had my
username but not the correct domain name in the subject header. What
was interesting is that Mail would not mark it as junk. I'd mark it as
junk, it would disappear from the inbox like usual, but when I looked in
the Junk mailbox it wasn't there. Going back to the Inbox, there it
was. I repeated this a bunch of times using the keyboard combination
and the menu to mark it as junk, and Mail.app just wouldn't do it. It
accepted marking all the other spams as junk, just not that one. Very
odd. I forwarded it to the spam trap at my ISP and trashed it, and then
belatedly thought I should examine the headers. Is there a header that
can be set which will prevent Mail.app from marking junk mail as junk?

This one is usually not fake. Some spammer is really using your email
address to send spam to other people. If the mail delivery fails, it
is you that the error report goes to!

I really doubt it. I'd say this was just a database glitch - next time
try using the "Rebuild Mailbox" menu option to see if you can click it
into gear.
